What you'll do

• Support individuals across multiple homes, including coordination and transportation to medical appointments
• assist with self-care and community activities
• document information according to Individual Plans
• help maintain clean and safe residences and vehicles
• when CNA responsibilities are met, cover open Direct Support Professional hours as needed.
