Lead Outreach Case Manager

Job overview

The Lead Outreach Case Manager role is onsite in Los Angeles, CA, with field work in Service Planning Area 6; pay is $27.81–$29.00 per hour. The role is part of the HOPICS division at Special Service for Groups and supports outreach to people experiencing homelessness. The case manager assesses immediate needs, coordinates services and treatment planning, and leads team activities and documentation. The work connects clients with housing, benefits, health, food, hygiene, employment, and other community resources while coordinating with a multidisciplinary team.

What you'll do

• Conduct immediate-needs assessments and contribute to treatment plans
• coordinate with multidisciplinary teams
• provide field-based case management and linkages to benefits, housing, health, substance-use, food, hygiene, employment, and education resources
• document services and outcomes in HMIS
• coordinate team activities and ensure accurate team records
• support required reports and meetings
• provide HIV/AIDS information and service linkages
• coordinate transportation as needed.
