What you'll do

• Review referrals and patient information to determine care needs
• assign hospice personnel
• evaluate cases through visits, conferences, and record review
• guide clinicians
• review diagnoses, prognosis, medications, and clinical course
• manage medical records and EMR workflow
• support care planning and case conferences
• conduct quarterly record reviews
• assist with hiring, orientation, and staff education
• participate in quality improvement activities.
