The Certified Home Health Aide (CHHA) – Home Health role is field-based, visiting patients in their homes in the Grass Valley, CA territory. Working under the direct supervision of an RN Case Manager, the aide provides personal care and other services according to each patient’s Plan of Care. The role supports patients’ daily needs and safety, performs assigned simple procedures, and reports condition changes to the assigned nurse.

What you'll do

• Provide personal care, including bathing, oral hygiene, shampoos, and changing bed linen
• assist with dressing, toileting, and feeding as needed
• provide skin care
• keep the patient’s living area orderly as appropriate
• perform ordered range-of-motion and other simple procedures when competency requirements are met
• assist with self-administration of medication
• use equipment safely
• report patient condition and significant changes to the assigned nurse.
