What you'll do

• Serve as a member of the Hospice team in assigned settings
• provide input to the Hospice Division and interdisciplinary team about patient interactions and care-plan changes
• document hours and services and maintain HIPAA confidentiality
• assist family caregivers with prepared and agreed-upon activities
• maintain availability for at least one hour per week
• participate in in-services as needed
• communicate with the Hospice Division or social worker
• respect patient rights
• carry out special projects as directed.
