What you'll do

• Perform physical and psychosocial assessments of hospice patients
• assess and reassess pain and use appropriate pain-management techniques
• educate patients and families about pain management
• assess patient condition and initiate, reevaluate, and update plans of care
• obtain physician orders and collaborate with physicians
• initiate case conferences on multidisciplinary responsibilities, pain management, and bereavement counseling.
