The Personal Service Coordinator / Intensive Case Manager — Housing & Mental Health role is an onsite, full-time position in Valencia, CA, paying $24.00 per hour for 35 hours per week. Mental Health America of Los Angeles provides integrated mental and physical health, housing, employment, and wellness services to people in the community. The role provides intensive case management to people eligible for permanent housing through the Department of Health Services Housing for Health program. It supports clients through outreach, enrollment, housing stabilization, service coordination, and documentation so they can move into and maintain permanent supportive housing.

What you'll do

• Manage a caseload of approximately 40 members
• conduct outreach, screening, intake, and enrollment
• assist with housing applications, move-ins, and stabilization
• complete psychosocial assessments and individualized service plans
• connect clients with health, mental health, substance use, benefits, employment, and other services
• support independent living and housing stability
• maintain required records
• participate in the on-call crisis prevention rotation.
