What you'll do

• Provide initial and annual nursing assessments
• develop nursing guidelines and tracking tools
• participate in interdisciplinary care
• coordinate hospital readmissions and follow-up care
• assess and triage health needs
• provide nursing interventions and document care
• coordinate routine and preventive care
• prepare medication records and clinical summaries
• train direct-care staff
• conduct medication and quality-assurance checks
• support regulatory and accreditation preparation.
