This position prioritizes engaging families experiencing homeless, often with individuals experiencing behavioral health challenges that may include mental illness, substance use, and/or intellectual or developmental disabilities. This role delivers street outreach to coordinate immediate shelter, provide case management, crisis intervention, and care coordination
