Case Manager

What you'll do

• Manage an assigned caseload and maintain regular youth contact
• coordinate with DCS, caregivers, residential staff, schools, therapists, medical providers, and support teams
• participate in treatment planning, team meetings, and discharge planning
• monitor behavioral, educational, medical, emotional, and independent-living needs
• complete case notes, assessments, summaries, and treatment-plan updates
• monitor appointments, medications, school participation, incidents, family contacts, and goals
• assist with crisis response and safety planning
• maintain compliant records
• advocate for youth needs.
