Hospice Care Consultant

The Hospice Care Consultant is an onsite role based in Faribault, MN. Compensation is provided in the range of $75,000 - $85,000 per year. This role supports Gentiva Hospice by building relationships with referral partners. This role exists to act as a strategic sales representative who champions the hospice mission. The Consultant educates healthcare professionals about hospice options and drives referral growth to ensure patients receive appropriate care.

What you'll do

• Educate, inform, and inspire confidence in hospice services.
• Identify new referral opportunities by calling on hospitals, home health agencies, skilled nursing facilities, assisted living communities, clinics, and physician offices.
• Engage in meaningful conversations with healthcare professionals about patient needs.
• Plan and execute strategic marketing initiatives to increase awareness and drive referral growth.
• Represent the organization at community events, professional associations, and educational in-services.
• Serve as a resource and advocate for patients and families.
