Direct Support Professional — Community Development Services

Job overview

The Direct Support Professional role is onsite in Baltimore, MD, with pay of $17 per hour and a Monday–Friday schedule from 8 a.m.–2 p.m. Helpful Hearts Health Services supports people with developmental disabilities in building independence, community integration, and meaningful connections. The professional provides person-centered support for community participation and life-skill development, helping individuals work toward goals in their individualized service plans while maintaining safety, dignity, and well-being.

What you'll do

• Facilitate community activities
• support communication, self-advocacy, employment readiness, and independent-living skills
• encourage volunteering, social groups, and recreation
• implement individualized service plans
• provide transportation to activities as needed
• maintain documentation and progress notes
• support individuals’ safety and dignity.

Why this role
