The RN provides individualized nursing care to clients in their homes, supporting their health needs through assessment, treatment, medication administration, education, and coordination with families, physicians, and the care team. PRN hours depend on client needs and availability and are not guaranteed.

What you'll do

• Assess and monitor client condition in the home
• promptly communicate changes or significant findings
• provide ordered skilled nursing treatments and medication administration
• support clients with chronic conditions, post-hospital needs, and ongoing nursing needs
• coordinate with physicians, families, and care team members
• educate clients and families
• complete timely clinical documentation in the WellSky app
• follow care plans and agency policies.
