Case Manager — Adult Behavioral Health

The Case Manager role is onsite in West Warwick, RI. Compensation is up to $20 per hour. Elwyn is a mission-driven nonprofit supporting people with autism, intellectual and developmental disabilities, and behavioral health challenges. The role provides daily care, counseling, and advocacy for young adult through geriatric clients with chronic mental illness and co-occurring disorders. Through recovery-based interventions, treatment planning, medication monitoring, and coordination of support, the case manager helps clients pursue social, economic, emotional, and personal independence in the community.

What you'll do

• Provide daily client care, counseling, and advocacy
• recognize mental illness symptoms and intervene therapeutically
• monitor prescribed medications under state-specific guidelines
• maintain client records
• develop and modify treatment plans based on assessments
• provide psychoeducation
• perform CPR, crisis intervention, and medication administration under agency protocols
• provide rotating on-call coverage.
