What you'll do

• Engage prospective care management participants
• conduct community outreach and home visits
• help individuals access transportation and community resources
• provide follow-up by phone or in person
• coordinate reminders for medication and medical appointments
• help secure referrals to medical, behavioral health, and social assistance providers
• provide status updates and document cases
• support socialization, recovery, self-advocacy, and community living skills
• travel and work in the field at least 50% of the time.
