What you'll do

• Build patient relationships and assess barriers to care
• connect patients with internal and community resources
• assist with appointments, coverage enrollment, and follow-up
• conduct community outreach and health education
• maintain confidential records and program data
• coordinate with multidisciplinary health center teams. Travel throughout the assigned service area and flexible evening or weekend work may be required.
