What you'll do

• Conduct initial and ongoing speech-language evaluations in patients’ homes
• develop, implement, and document individualized treatment plans for speech, language, cognitive, and swallowing disorders
• educate patients and caregivers on home programs and compensatory strategies
• coordinate with physicians, nurses, and other healthcare team members
• complete OASIS and EMR documentation
• communicate with the home health office team.

Why this role
