Medical Assistant

The Medical Assistant role is onsite in San Leandro, CA, with an anticipated salary range of $26–$30 per hour and a target annual bonus of 5%. Habitat Health’s PACE program provides coordinated medical and social care for older adults in centers and homes. The role supports participant-centered care by assisting with clinical procedures and documentation, coordinating follow-up activities, and working with providers and the Interdisciplinary Team to support safe, efficient care and continuity for participants.

What you'll do

• Complete intake and rooming, obtain vital signs and screenings, fulfill provider orders, and prepare participants for examinations
• perform routine procedures within scope, including laboratory testing, phlebotomy, EKGs, immunizations, injections, and continuous glucose monitoring
• communicate observations and status changes
• coordinate follow-up visits and assessments
• respond to participant questions and escalate requests as appropriate
• maintain EMR documentation, referrals, records, authorizations, clinic readiness, and inventory.
