What you'll do

• Provide brief and longer-term psychotherapy, consultation, assessment, and evaluation
• collaborate with the clinical practice team
• coordinate patient care and support self-management
• participate in projects addressing patient population needs
• maintain required documentation
• consult with medical providers and other programs
• provide patient advocacy, outreach, and education
• follow department protocols.
