What you'll do

• Provides quality Nutrition Care through the Nutrition Care Process.
• Completes Nutrition Assessment, Diagnosis, Interventions, and Monitoring.
• Provides Medical Nutrition Therapy and education to participants and caregivers.
• Reports changes in participant status to the Interdisciplinary Team.
• Conducts in-person re-assessments yearly and every six months.
• Participates in Interdisciplinary Team meetings.
• Monitors monthly weights, medical diagnosis, drug-nutrient interactions, and hydration status.
