Community Liaison/Caregiver

The Community Liaison/Caregiver is a hybrid, field-based role serving clients in Escondido, CA, with a regular schedule of Thursday through Monday and Tuesday and Wednesday off. The role combines non-medical caregiving with client visits, field operations, caregiver support, and office assistance. Direct care may account for up to all working hours when urgent, last-minute, or uncovered shifts need coverage, helping maintain continuity of care for clients.

What you'll do

• Provide non-medical personal care and support under individualized care plans
• accept assigned urgent or uncovered shifts
• assist with daily living activities, companionship, meal preparation, light housekeeping, laundry, and errands within authorized duties
• document care and report changes or concerns
• conduct client visits and check-ins
• support client and caregiver needs
• help maintain care plans, records, and agency documentation
• provide office and administrative support when not assigned to care or field duties.
