What you'll do
- Develop relationships with physicians, hospitals, case managers, social workers, managed care organizations, and other referral partners
- support census and payer-mix goals through outreach
- collaborate on marketing plans and review them at least quarterly
- track referrals and maintain CRM records
- represent the organization at community events and educational programs
- educate partners and staff about services
- coordinate clinical and financial assessments
- facilitate admissions with patients, families, referral partners, and facility staff
- coordinate room readiness, equipment, and transportation
- support insurance authorization
- conduct tours and respond to inquiries
- maintain referral and admission records
- assist families with initial long-term-care Medicaid information
- travel within the assigned service area.
