What you'll do

• Evaluate and treat communication, swallowing, and cognitive-linguistic disorders in patients’ homes. Conduct dysphagia and OASIS assessments
• develop individualized treatment plans
• provide skilled intervention for aphasia, dysarthria, dysphagia, and cognitive-communication deficits
• make MBSS/FEES referrals as appropriate
• coordinate with the care team
• educate patients and caregivers
• and maintain timely, accurate documentation.
