What you'll do

• Participate in initial assessments, care planning, and periodic reassessments
• document participant updates
• coordinate with interdisciplinary team members
• coordinate 24-hour care delivery
• provide individual and family counseling
• facilitate group counseling and activities
• refer participants to community resources
• conduct home visits
• participate in team meetings and quality improvement.
