Registered Nurse (RN) Case Manager – Home Health and Hospice

The Registered Nurse (RN) Case Manager – Home Health and Hospice role is an onsite, part-time position in Salt Lake City, UT, with compensation of $36–$45 per hour based on experience. The role primarily serves home health patients in the Salt Lake Valley, with occasional hospice visits and support as needed. The case manager provides skilled nursing care and coordinates individualized care so patients can remain as independent and comfortable as possible at home. The role works with patients, families, physicians, and other care-team members to monitor progress, adjust care plans, and support patient-centered care.

What you'll do

• Perform comprehensive nursing assessments and ongoing evaluations
• provide skilled nursing care including wound care, catheter care, venipuncture, medication management, and education
• develop individualized care goals and plans with patients, families, physicians, and care-team members
• coordinate care with healthcare providers
• monitor progress and adjust plans
• educate patients and families about medications, conditions, treatment plans, and resources
• supervise and direct care-team members
• maintain clinical documentation and regulatory compliance
• advocate for patients’ independence and comfort at home.
