What you'll do

• Provide supported education and employment services, case management, coordination, and referrals
• monitor clients in home and community settings
• teach education, employment, living, social, and coping skills
• provide crisis problem-solving assistance and follow-up
• maintain care plans, assessments, and documentation to applicable standards
• conduct community outreach with employers and schools.
