What you'll do

• Coordinate patient care with providers and health team members
• teach and advocate for patients and support informed treatment decisions and patient-desired independence. Conduct home visits to assess the home dialysis environment for safety and CMS compliance. Participate in continuing education, quality assessment and improvement, and clinical application of research.

Why this role

Requires two years of RN experience and at least two years of nephrology nursing with dialysis experience. Requires active, unencumbered RN licensure, CPR certification, and CNN/CDN certification or completion within 12 months. A bachelor’s degree is preferred.
