Description

Develops and implements discharge planning within an interdisciplinary healthcare team. Identifies and removes barriers that prevent optimum access to needed post-acute care. Works collaboratively with providers to identify discharge barriers and delays. Optimizes efficient use of acute care hospital days and prevents prolonged length of stay.
