Clinical Transition Coordinator

What you'll do

• Coordinate patient transitions to alternative care sites
• educate patients and families about illness, treatment, services, and financial responsibilities
• collaborate with physicians, case managers, sales, clinical, and operations teams
• support referral development
• document patient and payer information
• assist with therapy paperwork
• maintain territory and expense reports.
