What you'll do

• Assess patient suitability for home care and prescribed therapy
• develop patient-specific pharmacy plans
• recommend and document dosing
• enter and dispense prescriptions and supplies
• troubleshoot medication pumps
• document communications and patient progress
• ensure compounding compliance
• participate in patient-care conferences
• oversee and train pharmacy support personnel
• participate in the on-call process.
