What you'll do

• Engage shelter-residing families around housing, employment, income, health, transportation, childcare, documentation, and other service needs
• maintain timely client records, HMIS documentation, service plans, case notes, assessments, referrals, and housing documentation
• conduct care management sessions and housing-focused assessments
• travel locally for home visits, meetings, and appointments
• participate in occasional evening or weekend activities as required.
