What you'll do

• Oversee the Social Services department and resident psychosocial services
• complete assessments, documentation, and care planning
• coordinate discharge planning and connect residents and families with community resources
• participate in interdisciplinary care plan meetings
• support residents and families through changes in condition, placement needs, and other concerns
• maintain timely documentation
• collaborate with Nursing, Admissions, Therapy, and other departments
• and assist with survey preparation and regulatory compliance.
