What you'll do

• Conduct initial and periodic resident reviews
• monitor service delivery

• educate residents about available services, application procedures, and resident rights
• refer residents to volunteer support groups and community organizations
• help identify residents needing assistance and coordinate support
• maintain resident files and documentation
• respond to emergency requests as part of the response team.
