What you'll do

• Provide contracted case management services to a partial or reduced caseload in Anoka County
• conduct weekly one-to-ones with identified staff
• provide day-to-day staff support and consultation
• advise staff on policies and programs
• support audits, billing, and case management goals
• conduct training and shadow meetings as needed
• assure quality, person-centered services
• participate in or lead committees and focus groups as assigned.
