Community Care Coordinator

What you'll do

• Contact and enter new patients in the clinic’s tracking system
• provide patients and families with information and support before and after surgery
• coordinate AMPOWER visits
• build referral relationships
• assist with clinical documentation for insurance coverage
• maintain follow-up communications and appointments
• share updates from surgeon visits
• visit hospitals, skilled nursing, or rehabilitation facilities as needed
• follow up on referrals
• coordinate care among patients, providers, and clinic services. Assistance with fittings may require becoming an ABC Certified Orthotic Fitter.
