What you'll do

• Assess and monitor patients’ physical, emotional, and psychological needs
• create home-health care plans aligned with physician orders and patient goals
• administer medications, treatments, and interventions
• provide pain and symptom management
• educate and support families and caregivers
• collaborate with an interdisciplinary team
• maintain timely documentation
• participate in the local branch on-call rotation as required.

Why this role

Requires an accredited associate or bachelor’s nursing degree, current unrestricted state RN license, one year of nursing practice, valid driver’s license and car insurance, and current CPR; new graduates may be considered in select markets based on program availability.
