What you'll do

• Provide supports according to the Community Habilitation Plan
• help identify participant goals, preferences, strengths, and learning needs
• support meaningful activities and community participation
• teach and model skills using structured routines and least-restrictive prompting
• provide personal care as needed
• follow dietary, medical, and risk-management guidance
• use safety and crisis-prevention strategies
• document sessions and progress in the electronic medical record
• communicate with the participant’s support network and professionals
• identify community resources and assist with community-based activities and services.
