What you'll do

• Coordinate resident care assessment, planning, implementation, and evaluation through the MDS process
• facilitate interdisciplinary care conferences
• complete and review MDS assessments
• educate and assist staff with assessment and MDS processes
• implement MDS/PPS updates and provide staff in-service
• cross-train between PPS and OBRA
• conduct resident interviews
• respond to clinical emergencies and regulatory visits or issues outside normal hours as needed.
