Nurse Care Manager

What you'll do

• Coordinate care for high-risk patients
• develop comprehensive chronic-care plans
• conduct in-home and telephonic assessments
• perform medication reconciliation
• contact patients and caregivers within 48 hours of discharge
• coordinate services across providers, hospitals, and outpatient settings
• educate and advocate for patients and caregivers
• document and track care using EHR and care-management systems
• manage a patient caseload and identify gaps in care.
