The Certified Nursing Assistant (CNA) role is onsite and on the road in Sun Prairie, WI. Compensation is $18–$23 per hour. Moments Hospice’s team focuses on patient comfort and dignity during patients’ final days. The CNA provides personal care and daily-living assistance under the direction of a Case Manager RN. The role supports patient comfort through care-plan review, mobility and nutrition assistance, and accurate electronic medical records, while collaborating with the case manager.

What you'll do

• Provide personal care including bathing, dressing, grooming, toileting, and skin care
• review patient care plans
• assist with lifting and transfers, range of motion, positioning, exercise, and ambulation
• assist with dining and nutrition
• maintain electronic medical records
• work under the direction of a Case Manager RN.
