What you'll do

• Provide hands-on support in individuals’ homes and communities
• assist with personal care, meal preparation, and household tasks
• support community activities, appointments, and outings
• follow individualized care and behavior support plans
• administer medication when trained and authorized, where applicable
• document services according to New Jersey DDD requirements
• communicate with families, care team members, and agency staff about needs and progress
• maintain a safe, respectful, person-centered environment.
