What you'll do

• Coordinate patient care with care team members
• facilitate provider, physical therapy, and occupational therapy visits
• deliver medications and review instructions
• reconcile medication lists
• coordinate telehealth visits
• train patients on remote patient monitoring equipment
• deliver and assemble durable medical equipment and train patients and caregivers
• collect and label blood specimens and perform routine venipuncture as needed.
