Care Coordinator — Acute Care

The Care Coordinator — Acute Care works onsite at the Longview Campus in Fort Collins, CO. Pay is $24.04–$28.85 per hour. The role supports SummitStone Health Partners’ care coordination services for people transitioning from acute care settings. The coordinator helps clients pursue recovery goals through education, outreach, and connections to Medication Assisted Treatment and community supports. By coordinating with acute care teams and community partners, the role supports successful discharge, follow-up care, and continuity of services.

What you'll do

• Provide care coordination in collaboration with clients
• conduct referral-based and cold outreach
• engage emergency departments, primary care, acute care, crisis response, and behavioral health partners
• assist clients in navigating healthcare and community systems
• assess needs and coordinate post-discharge services
• identify resources and service gaps
• participate in Certified Addiction Specialist coursework
• collaborate with SummitStone staff and the CO-SLAW network.

Why this role

Schedule: Thursday–Saturday, 8:00 a.m.–8:30 p.m., and every other Wednesday, 8:00 a.m.–4:30 p.m. The role requires occasional travel to client homes and community partner sites.
