What you'll do

• Make home visits as delegated by the care-managing RN
• observe, record, and coordinate care with other disciplines regarding treatment and changes in condition
• provide personal care including bathing, showering, hair and nail care, and continence care
• perform light housekeeping such as preparing a light meal, changing linens, making beds, and emptying trash.
