What you'll do

• Complete nutrition assessments and recommend interventions
• update care plans
• monitor residents’ weight, intake, hydration, and nutritional status
• educate and counsel residents and families
• collaborate on clinical protocols and best practices
• participate in care planning meetings
• conduct quality assurance audits and follow up on corrective actions
• send visit summaries and invoices.
