What you'll do

• Assess patients and collaborate with the care team on plans of care

• teach self-care and support health and independence
• supervise LPNs and HHAs
• complete timely, accurate documentation
• communicate patient progress
• plan and provide staff education
• perform assigned on-call services
• participate in clinical development and continuing education.
