PRN Registered Nurse – Geriatric Client Assessments

The PRN Registered Nurse – Geriatric Client Assessments role is onsite in Dallas, TX, with in-home and facility-based visits. Pay is $43–$47 per hour. Overture Home Care supports seniors in the Dallas area, including people needing Alzheimer’s/dementia, hospice, and general geriatric care. The nurse conducts assessments as needed to determine care needs and eligibility, recommends appropriate services, and coordinates with intake, case management, nursing teams, clients, families, and referral sources. Accurate clinical documentation and support for care transitions and quality improvement help connect clients with suitable care.

What you'll do

• Conduct in-home and/or facility-based client assessments
• evaluate conditions and recommend levels of care and services
• coordinate with intake, case management, and nursing teams
• document assessments and clinical findings
• communicate with clients, families, and referral sources
• support care transitions
• maintain referral relationships
• provide clinical support to nursing and caregiving staff
• participate in case reviews and quality improvement
• travel within the Dallas area as required.
