Registered Nurse (RN) Care Manager

What you'll do

• Provide nursing services under the State Nurse Practice Act and agency policy
• review and sign off on caregiver skills and qualifications
• assess clients’ physical, psychosocial, and environmental status
• complete assessments within 48 hours of referral unless otherwise arranged
• develop or follow individualized plans of care
• provide skilled interventions
• report changes and update care plans
• teach clients
• communicate significant changes to physicians and team members
• maintain records and documentation
• participate in team meetings
• promote safety and infection control
• support caregiver development and supervise team members as delegated.
