The Clinical Support RN provides flexible coverage for the field team, helping maintain timely, high-quality patient care while giving other nurses dedicated time to complete documentation. The role delivers in-home skilled nursing, completes assessments and records, and coordinates with the care team to support patients’ safety, independence, and health outcomes.

What you'll do

• Provide one-to-one in-home skilled nursing visits for patients recovering from illness or surgery or managing chronic conditions
• complete OASIS assessments and documentation using Axxess Technology EMR
• collaborate with therapists, aides, and case managers
• educate patients and families
• maintain regulatory and professional practice standards.
