What you'll do

• Provide personal care and assist with activities of daily living according to the plan of care
• maintain a safe environment
• assist with ambulation, transfers, and range-of-motion exercises
• prepare nutritional snacks as appropriate
• under nurse supervision, assist with pre-poured medications in home care when applicable
• perform simple wound, skin, and bowel care under nurse direction where appropriate
• report changes in patient status
• document care and observations
• complete charting, timesheets, and mileage logs
• attend care conferences and relevant team meetings
• participate in scheduled on-call rotation for home care only.
